
To read our community's 10-year plan in detail, please see the attachment below. Due to the detail and depth of the plan, we have converted our 10-year plan into a Logic Model. A Logic Model is otherwise known as an Outcome-Based model. Outcome-based evaluation is a systematic way to assess the extent to which a program has achieved its intended results. It identifies the changes in the lives of individuals, families, organizations, or the community as a result of the program. To view the Logic Model version, see the attachment below.
